For your spinal problem, I would buy and use an Exped 9 Downmat and I would prefer a light synthetic sleeping bag over a fleece bag. My choice here is the Integral Designs Andromeda Overbag, rated to 40*F and very roomy while weighing about 2 lbs.

I own and use this combo and it is superbly comfortable.
